Real-world data from a 12-month remote weight management program found mean weight loss of roughly 22% at 12 months in the tirzepatide group and 17% in the semaglutide group, with at least 10% weight loss achieved by 95% of tirzepatide participants and 83% of semaglutide participants.

Semaglutide vs. Tirzepatide: What the Head-to-Head Data Shows

For years, clinicians compared these two medications across separate trials with different patient populations. That changed in 2025.

In the Phase 3b SURMOUNT-5 trial, tirzepatide demonstrated significantly greater weight loss compared with semaglutide in adults with obesity without diabetes. Over 72 weeks, participants on tirzepatide achieved a mean body weight reduction of 20.2% compared to 13.7% for those on semaglutide. Tirzepatide users were also more likely to achieve reductions of at least 10 to 25% in body weight and showed greater reductions in waist circumference.

Both medications produce significant results. Tirzepatide produces greater overall weight loss for many patients, but the right choice depends on your starting point, medical history and your healthcare provider’s recommendations.
